Forms and supporting documents for use by Catholic schools.

We provide model application forms and supporting documents for use by Catholic schools and academies.  The model application forms are available here. 

They are not to be downloaded from this site by job applicants. If you are applying for a position, please use the form provided by the prospective employer.

Applicants must not return any completed application forms and/or supplementary documents to the Catholic Education Service as they will not be processed. All completed application forms must be returned to the school or academy where the applicant applied, or as instructed in the details of the post.

Please note that the CES has worked with some recruiters who provide online application forms and has approved their online forms for use.  If you are unsure as to whether an online application form has been approved for use, please contact your diocese.

If you are unsure as to whether an online application form has been approved for use, please contact your diocese. To follow are the model documents which support the application forms. The notes to applicants should be provided as part of the recruitment pack along with the Consent to Obtain References Form, the Recruitment Monitoring Form and the Equality Act 2010 – Reasonable Adjustments Statement. The Rehabilitation of Offenders Act 1974 – Disclosure Form should only be sent to those candidates who are invited for interview. As advised in the recruitment guidance, schools and academies should also ensure that a copy of the relevant model contract is provided to each candidate invited for interview along with the diocesan guidance on the definition of practising Catholic if this is required. All of the documents below are available in download and amend format and schools and academies should adapt them as appropriate.
